Qualifications

• Licensed in Physical Therapy in the state of LA.
• Current CPR certification.
• Current driver's license with access to reliable transportation.

Responsibilities

• Evaluates patient's functional status and needs for CMS documentation.
• Develops and implements the PT plan of care in collaboration with the physician.
• Treats patients to relieve pain and restore function while maintaining standards.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients across the service area and submits progress notes per policy.
• Maintains licensure and participates in ongoing education and staff conferences.
