Job summary

UnitedHealth Group's Caretenders, part of LHC Group, is seeking a Physical Therapist to perform home health assessments and develop treatment plans to improve patient mobility and well-being.

You will implement interventions, document progress, and collaborate with the care team, while enjoying a flexible full-time weekday schedule and a comprehensive benefits package. A $10,000 sign-on bonus is available for external new hires.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists in plan of care development.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient response to treatment and any changes to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care, limitations, and progress toward goals.
• Visits patients, completes progress notes, and submits per policy.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
