Job summary

Concierge Home Care is seeking a Home Health Physical Therapist to deliver one-to-one care in patients' homes across Ocala/Marion County. You’ll evaluate and create individualized plans, guide therapeutic exercises, and collaborate with physicians. Expect autonomy with a flexible schedule and no mandatory weekends.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise PTAs and complete documentation using streamlined technology.
• Health insurance starts after 30 days; PTO accrues from day one.
• Flexible employment options: Modified full-time, PRN, or 1099.
