Job summary

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to deliver high-quality, in-home therapy in Marianna and surrounding Jackson County, FL. You will evaluate patients, develop individualized treatment plans, and provide mobility training with autonomy over your schedule.

The role emphasizes 1:1 patient care, flexible employment options (Full-Time, Modified Full-Time, PRN, or 1099), and the ability to earn through productivity with quarterly bonuses.

Qualifications

• Active Florida Physical Therapist license is required.
• Reliable transportation and valid driver's license.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs).
• Complete documentation using streamlined technology.
