Concierge Home Care in Marianna, FL is hiring a Home Health Physical Therapist (PT) to deliver 1:1 in-home care with a flexible schedule. You’ll evaluate patients, design individualized treatment plans, and help mobility and recovery in patients’ homes.

Qualifications

Active, unencumbered Florida Physical Therapist license.
Valid driver’s license and reliable transportation.

Responsibilities

Evaluate patients and develop individualized treatment plans.
Provide therapeutic exercises and mobility training.
Educate patients and caregivers on safety and recovery.
Collaborate with physicians and interdisciplinary teams.
Supervise Physical Therapist Assistants (PTAs).
Complete documentation using streamlined technology.
