Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for PRN, part-time, or full-time home health visits in the Annandale, VA area.

The PT will join a multidisciplinary team delivering rehabilitative care and skilled nursing to adult and geriatric clients in the comfort of their homes. Responsibilities include home visits, performing diagnostic assessments, wound care, and developing individualized care plans.

Qualifications

• Active Virginia Physical Therapist license.
• Graduation from CAPTE-accredited PT program.
• Ability to work independently.
• Strong interpersonal, organizational, and problem solving skills.
• Proficient with EMR systems.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, ambulation, strength and endurance, balance, and safety assessments.
• Implement wound care interventions using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and collaborate with nursing in the multidisciplinary plan.
• Educate and instruct clients, families, or representatives in rehabilitative care and activities for health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.
