BAYADA Home Health Care in Falls Church, VA is seeking an experienced Physical Therapist (PT) for PRN, part-time or full-time home health visits in the Arlington area. The role involves evaluating patients, creating care plans, and delivering skilled PT services in clients' homes.

You will collaborate with a multidisciplinary team, document findings on a tablet, and participate in ongoing training and career development. A valid VA PT license is required and EMR experience is preferred.

Formación

• Must hold a VA Physical Therapist-PT license.
• Graduation from an APTA/CAHEA approved PT program with transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation, strength and endurance, balance, and safety assessments.
• Implement wound care interventions to prevent wound formation and promote wound repair.
• Develop and implement individualized care plans, including manual therapies, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan and coordinate with nursing in the multidisciplinary plan.
• Educate clients and families in rehabilitative care and activities to promote health, safety and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.
