Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to deliver home-based PT services to patients in their residences. The role involves evaluating mobility, planning individualized therapy, and coordinating with physicians and caregivers to promote independence.

The position is field-based (per diem) with travel to patient homes. It requires a PT license in California, strong communication, organization, and the ability to work independently in patients’

Competenze

• PT degree (BS or MS) from an accredited program.
• Current California PT license.
• Minimum 2 years of professional PT experience.
• Home health or community PT experience preferred.
• Current CPR certification (AHA).
• Valid driver’s license and reliable vehicle.

Mansioni

• Provide physical therapy services per physician’s plan of care and state requirements.
• Assess patients’ functional status, mobility, strength, gait, ROM, and rehab potential.
• Develop and direct therapy programs; educate patients and caregivers.
• Coordinate with physicians and other health professionals to support patient recovery.
• Advise patients and families on therapy goals and use of appliances.
• Document progress and discharge planning; report changes to physician.
• Supervise PT Assistants as per policy and state regulations.
• Provide in-service education to staff as needed.
