Job summary

Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to provide in-home PT services on a per diem basis in California.

The role involves travel to patients' homes, performing assessments, developing therapy plans, and coordinating with physicians and caregivers to support recovery. Ideal candidates will have two years of PT experience, a current CA license, CPR certification, and a valid driver's license.

Qualifications

• Licensed Physical Therapist in California with an active license.
• Minimum of two years of qualified PT experience.
• Home health or community-based PT experience preferred.
• CPR certification (American Heart Association).
• Valid driver's license and access to a reliable vehicle.
• Strong verbal and written communication skills; well organized.

Responsibilities

• Provide PT services per physician's care plan and applicable licensure requirements.
• Assess patients' functional status, mobility, strength, gait, ROM, and rehab potential.
• Develop and direct PT programs; educate patients and caregivers.
• Coordinate with physicians and healthcare team to support recovery.
• Supervise PT assistants and provide in-service education as needed.
• Complete clinical and progress documentation; participate in discharge planning.
