Caringhand Home Health Solutions, Inc. seeks a compassionate Physical Therapist to provide in-home services in California. This per diem, field-based role travels to patient homes and is paid per visit, $160-$180. You’ll assess function, plan therapy, educate caregivers, and coordinate care with physicians.

The ideal candidate has 2+ years of PT experience, CA license, CPR certification, and strong communication. A valid driver's license and reliable vehicle are required.

Formación

• Bachelor's or master's degree in physical therapy from an appropriately accredited program.
• Current California Physical Therapist license issued by the Physical Therapy Board of California.
• At least two years of professional experience as a Physical Therapist.
• Previous community-based or home health physical therapy experience.
• Current CPR certification by American Heart Association.
• Valid driver's license and reliable vehicle, insured as required.
• Strong verbal and written communication skills.
• Good organizational skills and ability to manage patient care responsibilities.
• Ability to work independently in patients' home environments.

Responsabilidades

• Provide physical therapy services according to the physician's plan of care and California Act requirements.
• Assess and evaluate patients' functional status, mobility, gait, ROM, and rehabilitation potential.
• Develop and direct appropriate therapy programs; educate patients and caregivers.
• Coordinate with physicians and other health care professionals to support recovery.
• Supervise Physical Therapy Assistants per policy and state regulations.
• Instruct home health aides every two weeks when PT is the only skilled service.
• Prepare and submit clinical and progress summaries; participate in discharge planning.
