Caringhand Home Health Solutions, Inc. is seeking a compassionate Physical Therapist to deliver in-home PT services in California.

As a field-based, per diem role, you will assess, plan, and implement therapy programs in patients’ homes, coordinating with physicians and caregivers to promote independence and recovery. The ideal candidate has CA license, 2+ years of home health experience, CPR certification, and strong communication skills.

Competenze

• Bachelor’s or Master’s degree in physical therapy from an accredited program.
• Current California Physical Therapist license issued by the Physical Therapy Board of California.
• At least two years of professional experience as a Physical Therapist.
• Previous community-based or home health physical therapy experience.
• Current CPR certification by American Heart Association.
• Valid driver’s license and access to a reliable vehicle, insured as required.
• Strong verbal and written communication skills.
• Good organizational skills and ability to manage patient care responsibilities.
• Ability to work independently and provide therapy in patients’ home environments.

Mansioni

• Provide physical therapy services according to the physician’s plan of care and CA PT Act requirements.
• Assess patients’ therapeutic and functional status, including home environment.
• Evaluate strength, mobility, gait, ROM, and other functional abilities.
• Develop and direct therapy programs and participate in plan-of-care decisions.
• Instruct patients and families on use and care of therapeutic appliances.
• Educate patients, caregivers, and staff on therapy goals and interventions.
• Train patients in prosthetic device use when indicated.
• Document progress with summaries and discharge planning.
