Job summary

Caringhand Home Health Solutions, Inc. seeks a compassionate Physical Therapist to provide in-home therapy services on a per diem, field-based basis.

You will assess mobility, strength, gait, and function to create personalized treatment plans for patients in their homes. The ideal candidate holds a PT degree, a CA license, CPR certification, and at least two years in home health or community settings, with travel to patient homes and thorough documentation.

Qualifications

• Bachelor's or master's degree in physical therapy from an accredited program.
• Current California Physical Therapist license issued by the Physical Therapy Board of California.
• At least two years of professional experience as a Physical Therapist.
• Current CPR certification by American Heart Association.
• Valid driver's license and access to a reliable vehicle in good working order.

Responsibilities

• Provide physical therapy services according to the physician's plan of care and CA PT Act requirements.
• Assess and evaluate patients' therapeutic, rehabilitative, and functional status in the home.
• Develop and direct therapy programs; educate patients and caregivers.
• Coordinate with physicians and healthcare professionals to support recovery.
• Supervise Physical Therapy Assistants per policy and state regulations.
• Report progress and changes in condition to the physician.
• Identify needs for additional home health services and refer as needed.
