Job summary

PARS Therapy in West Columbia, TX is seeking a compassionate Physical Therapist to join our home health team, serving patients in their homes and neighborhoods.

The PT will evaluate functional abilities, develop individualized plans, and provide therapies to restore mobility and reduce pain, with emphasis on patient education and safety. TX license required; home health experience preferred.

Qualifications

• Active Physical Therapist license in TX
• Professional liability insurance coverage for TX license
• Graduate degree or higher in Physical Therapy
• Experience in home health or rehabilitative settings preferred
• Ability to adapt care approaches to patient needs
• Up-to-date knowledge of evidence-based treatment techniques
• Excellent communication skills and professional demeanor

Responsibilities

• Evaluate patients’ physical abilities in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety
• Monitor progress and adjust plans to achieve outcomes
• Accurately document evaluations, treatments, and progress notes
• Collaborate with physicians, nurses, and occupational therapists
• Conduct home safety assessments and prevent falls or re-injury
• Deliver compassionate care while empowering patient independence
