Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Livingston, TX. You will evaluate patients, develop individualized plans, and provide therapeutic interventions to improve mobility and reduce pain in patients’ homes.

Responsibilities include gait training, manual therapy, patient and caregiver education, and progress documentation. A TX PT license, liability coverage, and a degree from an accredited program are required, with home health experience

Qualifications

• Active Texas Physical Therapist license.
• Professional liability insurance coverage for Texas PT license.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide exercises, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes in digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and suggest fall-prevention strategies.
• Deliver compassionate, patient-centered care to empower independence.
