UnitedHealth Group, through Cambridge Home Health (LHC Group), seeks a Physical Therapist to assess and treat patients in home health settings. You will evaluate functional status, develop plans of care, and deliver interventions aimed at improving mobility and comfort while coordinating with the care team.

The role offers flexible scheduling, including a 4-day work week, and a comprehensive benefits package.

Formación

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification (preferred; can be completed within 90 days of hire)

Responsabilidades

• Evaluates the patient's functional status and physical therapy needs.
• Assists with development of the plan of care.
• Provides services within scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patients and family on plan of care and progress.
• Completes evaluations and documents progress notes per policy.
• Reports outcomes and anticipated frequency of care.
