Job summary

The Home Health Physical Therapist at UnitedHealth Group evaluates patient care needs related to mobility and functional status in home settings across Arkansas. You will determine treatment plans, perform interventions to improve wellbeing, and monitor progress.

Primary responsibilities include assessments, plan of care development, and progress documentation, with coordination among care teams. A $5,000 bonus is available; a 4-day work week option may be available for full-time roles.

Qualifications

• Arkansas state licensure required.
• Current CPR certification required.
• Valid driver's license or access to reliable transportation.

Responsibilities

• Evaluates patient's functional status and PT needs; assists in development of therapy plan of care.
• Provides services within scope of practice and plan of care; coordinates with health care team.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain and restore function; maintains maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and completes progress notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
