Job summary

Elite Home Health, a part of LHC Group within UnitedHealth Group, is hiring a Physical Therapist to perform home health assessments and develop treatment plans tailored to mobility and functional status. You will deliver interventions to improve well-being, document progress, and collaborate with nurses and physicians.

A 4-day work week option and a $5,000 bonus for full-time positions are offered. Qualifications include licensed PT in the state, CPR, reliable transportation, and ability to work

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver's license with dependable vehicle.
• Ability to work in patients' homes and diverse settings.

Responsibilities

• Evaluates patient’s functional status and PT needs.
• Develops therapy plan of care in collaboration with physician.
• Provides PT services per state practice laws and plan of care.
• Observes and reports patient progress to care team.
• Instructures patients and families about plan of care and progress.
• Completes evaluations and progress notes per policy.
• Reports outcomes and projected frequency of care.
