Job summary

Ohio Living Village Home Health & Hospice is seeking a dedicated Physical Therapist to provide in-home evaluations and treatments under physician orders. You will participate in OASIS-based planning, collaborate with an interdisciplinary team, and guide patients toward maximum function in their home environment.

New graduates welcome, with preference for those experienced in geriatric care and home health modalities.

Qualifications

• Current unencumbered license for Ohio to practice as a Physical Therapist (PT).
• One year experience as a PT in hospital, long-term care, or home health preferred.
• New graduates welcome.
• Experience with geriatric patients and PT modalities.
• English language proficiency required.
• Valid driver's license and insurable driving record.

Responsibilities

• Perform PT evaluations and treatments in patients' homes as ordered by physicians.
• Develop and revise Plan of Care and discharge plans in collaboration with the care team.
• Educate patients and families on exercises, safety, and self-care activities.
• Communicate progress with physicians, families, and health team members.
• Maintain accurate patient records and comply with regulations.
