Qualifications

• Bachelor’s Degree in Physical Therapy and PA licensure.
• Ability to travel to Home Care patient care assignments.
• Excellent clinical reasoning and patient communication.
• Current American Heart Association Basic Life Support (BLS).
• PA license required upon hire.

Responsibilities

• Establishes and implements plans of care to restore function.
• Implements an individualized treatment plan using evidence-based techniques.
• Provides PT treatment aligned with patient needs assessment.
• Performs reassessment and modifies plans as needed.
• Communicates progress to supervisor, physicians, and team.
• Implements a patient/family directed approach with age/population needs.
• Provides supervision per regulatory requirements.
• Establishes measurable goals and timelines in care plans.
• Ensures documentation complies with policies and standards.
