Job summary

Lehigh Valley Health Network (LVHN) is seeking experienced Physical Therapists to join its Home Care team serving Allentown and surrounding territories. Evaluates, plans, directs, and administers PT programs to restore function, prevent disability, and help patients reach independence.

Requires a Bachelor’s Degree in Physical Therapy and PA licensure; BLS certification preferred; ability to travel to patient care assignments is expected.

Qualifications

• Bachelor’s degree in physical therapy and licensure in Pennsylvania.
• Must have current PT license (PA) at hire.
• Excellent clinical reasoning and communication with patients and families.

Responsibilities

• Evaluates, plans, directs, and administers physical therapy programs to restore function.
• Develops and updates a plan of care in collaboration with patient/family.
• Documents all treatment and communicates progress to the care team.
