Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited PT program.
• One year of related PT experience for home health.
• If <1 year, mentoring/ residency may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
