Qualifications

• Licensed to practice physical therapy in the state assigned.
• Graduate from a CAPTE-accredited PT program.
• Minimum 1 year home health PT experience preferred.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Perform reexaminations at least every 30 days and plan discharges.
• Provide interventions and modify plans as needed.
