Job summary

Compassus is seeking a Home Health Physical Therapist to deliver coordinated care across patient ages, planning and evaluating plans of care with the treating physician and IDT. The role focuses on restoring or maintaining patient well-being through targeted PT interventions and patient education.

The position emphasizes infection control, accurate documentation, and collaboration with the care team. A license, CPR, and active state credentials are required, with preferred acute care or rehab

Qualifications

• Bachelor of Science in Physical Therapy is required.
• Two years of experience as a Physical Therapist in acute care or rehabilitation setting is preferred.
• Active Physical Therapist license in state of employment is required.
• Current CPR certification is required.

Responsibilities

• Provides care utilizing infection control measures.
• Prepares and completes a Plan of Care in consultation with the physician and care-team members.
• Performs initial and ongoing clinical assessments, including OASIS assessments where appropriate.
• Collaborates with interdisciplinary team (IDT) to coordinate services.
• Performs and oversees treatment for neuromuscular, neurological, and orthopedic disorders.
• Instructs on transfer techniques and use of equipment ( walkers, wheelchairs, canes ) for patient safety.
• Documents progress notes and updates to medical records.
