Advocate Health is seeking a Physical Therapist for Home Health serving the Far Southwest Suburbs, focusing on Midlothian, Crestwood, Robbins, Blue Island, Calumet Park and Alsip. Standard M-F days, 8a–5p with weekend rotation and one summer/one winter holiday.

Eligible for a sign-on bonus up to $10,000. Licensed in Illinois or eligible for licensure, with at least 1 year of clinical experience or equivalent affiliation.

Qualifikationen

• Licensed in the State of Illinois as Physical Therapist
• 1 year of clinical experience or demonstrated strong clinical affiliation experience
• Proficient in documenting for reimbursement and regulatory compliance

Aufgaben

• Facilitates quality of care aligned with Advocate MVP and stewardship.
• Participates in staff meetings and educational activities to support organizational initiatives.
• Collaborates with care team to formulate an integrated approach to patient management.
• Performs patient assessments and documents care plans following evidence-based standards.
• Leads discharge planning and referrals as appropriate within interdisciplinary teams.
• Provides patient and family education to support self-care and safety at home.
