Job summary

Advocate Health Care is seeking a Physical Therapist for home health case management in the Chicago Southeast Side. This full-time role involves day hours with a weekend rotation and occasional holiday coverage, with a sign-on bonus of up to $10,000.

Qualifications

• Master's Degree in Physical Therapy and IL licensure (or eligible).
• BLS for Healthcare Providers certification to be obtained within 6 months.
• Ability to learn scheduling/documentation systems and maintain records.

Responsibilities

• Ensure quality of care with evidence-based practices and accurate documentation.
• Coordinate care as part of the patient care team and manage referrals.
• Lead discharge planning and interdisciplinary case conferences as appropriate.
• Provide patient and family education and ensure safe home exercise programs.
• Communicate effectively with the care team and manage workload assignments.
