Job summary

Advocate Health is seeking a Home Health Physical Therapist Case Manager for the southeast Chicago area. This full-time role requires 40 hours per week, with a schedule from 8:00 AM to 5:00 PM on weekdays and rotating weekend coverage. A $10,000 sign-on bonus is available for qualified internal applicants.

Salary range is $44.15-$66.25 per hour, with comprehensive benefits. Requires IL license, Master's in PT, and BLS certification.

Qualifications

• Master's Degree in Physical Therapy required.
• Licensed in the State of Illinois (or eligible for licensure).
• BLS for Healthcare Providers certification required within 6 months.

Responsibilities

• Facilitates quality of care and safeguards patient outcomes.
• Provides care coordination and interdisciplinary collaboration.
• Documents thoroughly to support reimbursement and regulatory requirements.
• Participates in weekend rotation and holiday coverage as needed.
