Job summary

Advocate Health at Home seeks a dedicated Physical Therapist to manage a home health caseload in Chicago. You will perform assessments, design individualized plans, and deliver in-home therapy to improve mobility, safety, and independence within a supportive, interdisciplinary team environment.

Role offers a daytime schedule with rotating weekends/holidays, travel across the North Side territory, and a sign-on bonus for external candidates.

Qualifications

• Licensed Physical Therapist in Illinois (or eligible).
• Minimum 1 year of clinical experience (or strong clinical affiliation).
• Experience with home-based therapy equipment and assistive devices.
• Strong communication, documentation, and time management skills.
• Ability to work independently while collaborating with a broader care team.

Responsibilities

• Provide comprehensive physical therapy assessments and develop individualized care plans.
• Deliver in-home therapy focused on improving mobility, safety, and independence.
• Serve as a case manager, coordinating care with physicians and interdisciplinary teams.
• Educate patients and families to support recovery and long-term outcomes.
• Travel throughout the assigned North Side Chicago territory.
