A leading home health agency is seeking a Part-Time Physical Therapist Case Manager to evaluate and treat patients in their homes. Ideal candidates will have a California Physical Therapist License and a passion for improving patient lives. The position offers flexibility in scheduling, competitive pay on a per-visit basis ranging from $100 to $150, and a supportive work environment including outstanding benefits for you and your family.

Formación

• Knowledge of local community resources is required.
• Experience in home health care is highly preferred.
• Understanding of individual development and human behavior.

Responsabilidades

• Evaluate and treat patients using medically prescribed physical therapy programs.
• Develop a care plan in collaboration with other clinicians.
