Job summary

Andwell Health Partners in Scarborough, ME is seeking a Physical Therapist Case Manager to deliver home health PT services, including evaluation, direct care, and care coordination in clients' homes and communities. You will develop and revise plans of care to promote independence, collaborate with families, physicians, and pay sources, and support outcomes aligned with client needs.

A Maine PT license and CPR are required; 1 year home health experience preferred; a reliable car with insurance

Qualifications

What You'll Do

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction.
• Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
• Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client's age, condition, and needs.
• Implement client care plan activities based on treatment goals towards independent living for the client/family.

What You'll Bring

• Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR card required.
• 1-year Physical Therapy experience preferred. Home health experience preferred.
• A reliable car with insurance.
