Job summary

Andwell Health Partners in Maine is seeking a licensed Physical Therapist to serve clients in their homes and local communities. You will assess, treat, and educate patients, coordinating with families and care providers to promote independence.

Qualifications

• PT degree from an accredited institution.
• Current Maine PT license.
• Current CPR card.

Responsibilities

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association.
• Coordinate client home care needs with client, family, physician, other providers and pay sources toward independence and well-being.
• Develop, implement, coordinate, and revise a plan of care aligned with client goals, age, condition and needs.
• Implement care plan activities based on treatment goals toward independent living for client/family.

Services may include screening, evaluation, direct service, consultation and/or instruction.Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.Implement client care plan activities based on treatment goals towards independent living for the client/family.What You’ll Bring:Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.Current PT license in the State of Maine.Current CPR card required.1-year Physical Therapy experience preferred.
