Job summary

Optum is seeking a Physical Therapist (PT) for home health in New Jersey. Responsibilities include evaluating patients, developing and implementing a PT plan of care, and coordinating with physicians and family/caregivers to improve function and reduce pain.

Role offers per-visit and hourly pay, with a comprehensive benefits package and growth opportunities within the Optum family. Applicants must have current PT licensure and a valid driver’s license, with CPR certification preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and needs; assists in developing the therapy plan of care.
• Provides PT services within the plan of care and state scope of practice.
• Observes and reports patient responses to treatment to supervisor/physician.
• Treats patients to relieve pain and restore function while maintaining standards of practice.
• Instructs patient, family, and health team about plan of care and progress toward goals.
• Visits patients per Plan of Treatment; submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and planned frequency of care.
• Functioning in diverse home environments regardless of demographics.
