Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance or reliable transportation.
• Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluate the patient's functional status and treatment needs per the plan of care.
• Provide PT services within the scope of practice as defined by state laws and the plan of care.
• Observe and report patient responses to treatment and any changes to supervising staff or physician.
• Treat patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instruct and inform the care team, patient, and family about the plan of care and progress toward goals.
• Visit patients per the Plan of Treatment and document progress notes per policy.
• Complete evaluations and develop the PT plan of care within state guidelines.
