Job summary

Generations Home Health, part of the Optum family, is seeking a Licensed Physical Therapist to join our home health team. You will assess patients’ functional status, develop and implement therapy plans, and collaborate with physicians and nurses to improve mobility and quality of life.

You will visit patients in their homes, document progress, and adjust care plans within state guidelines. A valid PT license, driver’s license, and CPR certification (preferred) are required or obtainable within

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient's functional status and develops therapy plan with physician.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes evaluations and PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Work effectively in diverse home settings.
