Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will help patients regain strength and independence through personalized, functional therapy.

Responsibilities include delivering the treatment plan, performing active and passive therapeutic exercises, documenting records weekly, attending educational meetings, and ensuring infection control and safety.

Qualifications

• Active PT license is required.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Experience collaborating with healthcare professionals.
• Geriatric population knowledge and care.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes as developed by the PT.
• Deliver non-complex active and passive exercises, manual therapy, and modalities as prescribed.
• Document client records and submit weekly clinical documentation.
• Attend educational meetings and departmental in-services.
• Notify PT or Branch Director of equipment or staff changes and ensure infection control.
