CenterWell Home Health seeks a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will administer the prescribed PT plan and deliver non-complex manual therapies to improve strength, mobility and independence.

You will document care, coordinate with the PT and Branch Director, and participate in team education. A valid PT license, driver’s license, CPR, and geriatric knowledge are required.

Qualifications

• Must hold current state Physical Therapy license.
• Valid driver's license and reliable transportation required.
• Current CPR certification preferred.
• Experience collaborating with a team of healthcare professionals.
• Solid knowledge of geriatric physical, emotional, social and biological changes.

Responsabilités

• Administer the physical therapy treatment plan in patients' homes.
• Perform non-complex active and passive manual therapy, modalities as directed.
• Document client records and submit on time weekly.
• Attend departmental meetings and in‑services.
• Notify PT or Branch Director of equipment or supply issues.
• Apply infection control and universal precautions in care activities.

Connaissances

Current state license to practice PT

Valid driver's license

Current CPR certification

Experience collaborating with health‑c

Geriatric population knowledge

Fluent English
