Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will help patients regain strength, mobility and independence, focusing on functional improvement and quality of life.

Ideal candidates will have a current state license to practice Physical Therapy, valid driver’s license and CPR certification, plus strong teamwork and geriatrics knowledge.

Qualifications

• Current state license to practice Physical Therapy (PTA license)
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of the physical, emotional, social, and biological changes in the geriatric population
• Read, write, and speak fluent English

Responsibilities

• Administer home-based physical therapy per plan developed by the PT
• Deliver non-complex manual therapy and therapeutic exercises
• Document client records and submit clinical documentation weekly
• Attend educational meetings and coordinate department in-services
• Notify PT or Branch Director of equipment issues or staff or client changes
• Apply infection control and universal precautions in care activities
