CenterWell Home Health in the United States is seeking a Home Health Physical Therapist Assistant to help patients regain strength, mobility and independence, at their homes, under the supervision of a Physical Therapist. You will administer the treatment plan and document progress as part of a coordinated care team.

Responsibilities include administering exercises, modalities, and documentation; coordinating with the Physical Therapist and Branch Director; ensuring infection control and safety.

Competenze

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric physical, emotional and social changes.
• Read, write, and speak fluent English.

Mansioni

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in patient care.
