Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to administer the prescribed physical therapy plan in patients' homes under the supervision of a Physical Therapist. You will work onsite in patients' homes, helping them regain strength, mobility and independence.

Responsibilities include documenting records, coordinating with the team, attending department in-services, and ensuring infection control.

Qualifications

• Must hold a current state license to practice Physical Therapy.
• Must have a valid driver's license and reliable transportation.
• Current CPR certification is required.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric population changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex manual therapies and modalities.
• Document client records and submit on time weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
