Job summary

Qualifications

• Current New Jersey Physical Therapist license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA/CAHEA/AMA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Conduct mobility and safety assessments during visits.
• Implement wound care and rehabilitation interventions.
• Develop and revise individualized care plans with patient and family.
• Educate clients and families on home exercise programs.
• Document observations and interventions accurately.
