Job summary
BAYADA Home Health Care in Elizabeth, New Jersey, is seeking a Home Health Physical Therapist for ongoing home visits with adult and senior clients in Union County. One year of PT experience and NJ PT licensure are required. Sign-on bonus of $20,000 is offered with competitive per-visit pay.
You will assess patients at home, develop and adjust care plans, and collaborate with a multi-disciplinary team to support safe, independent living.
Qualifications
- A current New Jersey Physical Therapist license.
- Minimum of one year of recent work experience as a PT.
- Graduation from an APTA-approved PT program.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements of mobility, strength, balance, and safety.
- Implement wound care interventions with evidence-based dressings.
- Develop and implement individualized care plans including therapeutic exercises and gait training.
- Continuously assess and revise the care plan with nursing input.
- Educate clients and families on rehabilitative care to promote independence.
- Document observations and interventions in an EMR on a tablet.
