Qualifications

• Active New Jersey Physical Therapist license.
• Minimum of one year of PT experience.
• Graduate from an American Physical Therapy Association-approved program.

• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, safety).
• Implement wound care and protective interventions as needed.
• Develop and implement individualized care plans including exercises.
• Reassess and revise care plans with nursing and other team members.
• Educate clients and families in rehabilitative care and activities for health and safety.
