BAYADA Home Health Care is seeking a Home Health Physical Therapist for a job in Perth Amboy, New Jersey. One year of PT experience and NJ license required. This full-time role involves home health visits across Middlesex County with a $20,000 sign-on bonus.

You will assess, plan, and implement individualized PT care, document in EMR, and collaborate with a multidisciplinary team to help clients stay safely at home.

Qualifications

• A current New Jersey PT license.
• At least one year of recent work experience as a Physical Therapist.
• Graduation from an APA-approved program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility, gait, strength, balance, and safety.
• Develop and implement appropriate individualized care plans with manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan.
• Educate and instruct clients and family in rehabilitative care and activities to promote health and independence.
• Accurately document observations, interventions, and evaluations in the EMR using a touch pad tablet.
