Job summary

Cantex seeks a licensed Physical Therapist to provide in-home health services across Beaumont, TX. You will evaluate, treat, and manage therapy plans to improve mobility and independence, while coordinating with an interdisciplinary team.

You will complete SOC/recertifications, supervise PTAs, document accurately, and participate in QAPI activities. A $10,000 sign-on bonus is offered along with a competitive benefits package and 401(k) with match.

Qualifications

• Licensed PT in Texas.
• At least 1 year in home health or similar setting.
• Strong knowledge of OASIS documentation and home health regulations.
• Excellent communication with patients, families, and care teams.
• Ability to work independently in a field-based environment.

Responsibilities

• Evaluate, treat, and manage home health patients to improve mobility and independence.
• Complete SOC, Recertification, and re-evaluation visits per physician orders and regulations.
• Develop and implement Plans of Care based on patient needs.
• Supervise PTAs in accordance with regulations.
• Coordinate therapy services with SN, OT, ST, MSW, and HHA.
• Monitor progress and modify treatments to meet functional outcomes.
• Educate patients and caregivers on exercises and safety.
• Maintain accurate clinical documentation including OASIS.
• Participate in care coordination activities and case conferences.
• Assist with discharge planning and transition of care activities.
• Participate in QAPI activities as assigned.
