TheraCare in Beaumont, TX is seeking a licensed Physical Therapist (PT) for a full-time home health role to provide one-on-one, in-home physical therapy to adult and geriatric patients across Beaumont and surrounding areas.

The position offers flexible daytime scheduling, clinical autonomy, and meaningful patient impact, including mobility and safety improvements. A $10,000 sign-on bonus is included, with competitive compensation and benefits.

Qualifikationen

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligibility for Compact Licensure.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Knowledge of OASIS and home health regulations preferred.
• Valid driver’s license and reliable transportation.

Aufgaben

• Perform home health PT evaluations and re-evaluations, including OASIS assessments.
• Develop and update Plans of Care based on goals and orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post-surgical conditions.
• Deliver interventions focusing on mobility, gait, balance, strengthening, and fall prevention.
• Apply therapeutic techniques and modalities as ordered by physicians.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and modify treatment plans as needed.
• Document accurately in the EMR at the point of care.
• Collaborate with physicians, nurses, case managers, PTAs, aides, and teams.
• Supervise PTAs and aides per state regulations.
• Participate in case conferences, discharge planning, and care coordination.
• Ensure compliance with home health regulations and infection control.
