Qualifications

• Doctorate in Physical Therapy from an APTA-accredited program.
• Active PT license in Texas or eligible Compact PT Licensure.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.

Responsibilities

• Perform home health PT evaluations and re‑evaluations, including OASIS assessments.
• Develop, implement, and update Plans of Care based on goals and orders.
• Provide skilled PT treatment for orthopedic, neurological, cardiac, post‑surgical conditions.
• Deliver interventions focused on mobility, gait, balance, and functional independence.
• Educate patients and caregivers on home exercise programs and safety.
