Job summary

Cantex Continuing Care Network is seeking a licensed Physical Therapist (TX) to evaluate, treat, and manage patients in home health settings. The PT will develop and implement individualized plans of care, supervise PTAs, and coordinate with an interdisciplinary team to ensure high-quality care and successful patient outcomes.

The role emphasizes accurate documentation, regulatory compliance, and patient/ caregiver education to support recovery, discharge planning, and continuity of care.

Qualifications

• Licensed Physical Therapist (TX) with strong knowledge of home health regulations.
• Experience evaluating, treating, and documenting in home health settings.
• Familiarity with OASIS documentation and therapy care planning.
• Ability to supervise PTAs and coordinate with interdisciplinary teams.

Responsibilities

• Evaluate and treat home health patients to improve mobility and independence.
• Complete SOC, recertifications, and re-evaluations per orders and regulations.
• Develop Plans of Care based on patient needs and physician direction.
• Supervise PTAs according to regulations and standards.
• Coordinate therapy with SN, OT, ST, MSW, and HHA.
• Monitor progress and adjust interventions for measurable outcomes.
• Educate patients and caregivers on exercises, safety, and independence.
• Maintain accurate clinical documentation including OASIS.
