Job summary

Next Move Healthcare in Kenedy, Texas seeks a licensed Physical Therapist (PT) to join our home health rehabilitation team. This in-home role focuses on evaluating and treating patients to improve mobility, strength, balance, and independence.

Work Monday through Friday, 8:00 AM–5:00 PM, with a starting pay of $45.00 per hour ($93,600 annually for a 40-hour week) and a $10,000 sign-on bonus. A comprehensive benefits package is included; Texas PT licensure and BLS are required; previous home

Qualifications

• Active Physical Therapist license in Texas.
• Ability to evaluate and treat in home health settings.
• Competent patient and family education and documentation.

Responsibilities

• Provide comprehensive PT evaluations and treatments in patients' homes.
• Assess strength, balance, ROM, gait, transfers, and functional limits.
• Develop and implement individualized home treatment plans.
• Deliver exercises, mobility training, balance activities, gait training, and rehab.
• Assess home environments for safety and fall risk; provide education.
• Coordinate with physicians, nurses, SLPs, and OT for care.
• Document evaluations, progress notes, and discharge summaries.
• Schedule visits and manage daily caseload for efficient service.
• Adhere to infection control and Texas/federal regulations.
