Job summary

Optum is seeking a licensed Physical Therapist Assistant for home health, to implement the plan of care under the supervision of a Physical Therapist and support patient well-being. You will follow treatment plans and educate patients and families.

Responsibilities include performing prescribed exercises, recording progress, coordinating with the care team, and ensuring safety in diverse home settings. Weekend coverage is required in this role.

Qualifications

• Current, licensed PTA in state of practice.
• Valid driver's license and reliable transportation.
• Willingness to work weekend rotations.

Responsibilities

• Follow state PT laws and plan of care under supervising PT.
• Instruct patients in active/passive exercises and gait training.
• Observe and report patient responses to treatment.
• Coordinate care with the health team.
• Educate patient, family, and caregivers on therapy.
• Work in diverse home settings, maintaining safety and care quality.
