Job summary

Optum is seeking a Physical Therapist Assistant to deliver therapy in a home-care setting and educate patients to improve well-being under the supervising Physical Therapist plan of care.

Responsibilities include exercising instruction, patient progress documentation, and coordinating with the care team. Strong focus on safety, adaptability to home environments, and CPR readiness is expected.

Qualifications

• Current, licensed, registered, or certified to assist in physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws governing provision of physical therapy in home care and adhere to plan of care.
• Instruct and aid patients in active and passive exercise, gait, ADL, and mobility training.
• Observe, record, and report patient responses to treatment to supervising PT and others.
• Coordinate care with other healthcare team members as appropriate.
