Job summary

The North Country Home Health & Hospice Agency is seeking a Physical Therapist Assistant to provide in-home physical therapy under the supervision of a licensed Physical Therapist. You will use therapeutic exercise, modalities and activity to achieve functional goals in the patient’s home.

The ideal candidate holds an accredited PTA credential and NH license, possesses strong organizational and communication skills, and can maintain detailed documentation.

Qualifications

• Associate degree from an accredited PTA program.
• Current NH PTA license.
• Good organizational and multitasking abilities.
• Excellent customer service and communication skills.
• Computer literacy: Windows.
• BLS certification required.

Responsibilities

• Performs PTA procedures in the client’s home under supervising PT.
• Administers therapeutic exercises and modalities as prescribed.
• Instructs and assists clients in functional activities and device use.
• Maintains legible SOAP notes and progress documentation.
• Ensures equipment is safe and reports unsafe conditions.
